When you purchase 10 bottles of HYLO® in 2020, you’ll get 1 free bottle of your favourite HYLO® dry eye treatment!

To get your free bottle of HYLO® by mail, please follow the steps below:

 

  • Purchase a total of ten (10) individual HYLO® dry eye treatments at valid* retail locations in Canada between January 1 and December 31, 2020. You can find details about the nearest HYLO® retailer to you here.
  • Complete the redemption form. Please fill out all sections of the form and print a copy to include with your submission for your free bottle.
  • Mail the original barcodes cut from the bottom flap of each retail box (showing the barcode, expiry date and lot number), along with all original purchase receipts and a copy of your completed redemption form to:

Next steps:

 

  • Once you’ve completed the redemption form online, you’ll get a confirmation email welcoming you to the 2020 Candorvision Patient Program.
  • Please check your junk/spam emails or add patients@candorvision.com to your contact list.
  • When your submission is received by mail, we’ll review it and then mail your bottle of HYLO® free of charge to the Canadian mailing address you provide on the redemption form.

How to prepare the bottom box flaps (with barcodes):

To provide the barcodes from the bottom box flaps for your submission please do the following:

  • Turn over the empty box
  • Undo the bottom flap which shows the barcode, expiry date and lot number.
  • Cut off the bottom flap along the seam to detach it from the box.
  • Keep the bottom flap (which should show the barcode, expiry date and lot number) in a safe place with your original proof of purchase receipt until you’ve collected a total of ten (10).

Collect ten (10) original barcodes by purchasing HYLO®, HYLO®GEL or HYLO®-DUAL in Canada at valid retailers such as pharmacies or clinics between January 1st 2020 to December 31st 2020. What happens after I mail my submission?

Once we receive your submission by mail, you’ll get a confirmation email from us. We’ll send you your bottle of HYLO® as soon as possible, free of charge (Candorvision will cover the cost of shipping).

When will I get my free bottle of HYLO®?

You’ll receive your free bottle of HYLO® as soon as possible after we receive your submission, within a maximum of 6-8 weeks depending on the volume of submissions. You’ll know we received your submission when we send you a confirmation email. Please check your junk/spam emails or add patients@candorvision.com to your contact list. To ensure your submission is processed as quickly as possible, please check the steps for how to get your free bottle before mailing in your submission.

Where can I buy HYLO®, HYLO®GEL and HYLO®-DUAL?

Our treatments are available behind the counter at most pharmacies and some clinics. You can look up the closest retailer to you here: https://candorvision.com/hylo/buy/.

Can I use photos or photocopies of the barcodes and receipts?

This patient program requires the original bottom flap of the retail boxes (showing the barcode, expiry date and lot number) as well as original proof of purchase receipts and the completed redemption form. I lost one of my receipts/barcodes. What do I do?

If you lost a receipt or bottom flap of one of your boxes, please contact us at patients@candorvision.com or (514)380-5270 before mailing your submission.

My receipt has confidential information on it, how do I protect my privacy?

You may modify your receipts to hide payment information and other purchases, so long as any information relevant to this Patient Program remains visible. Receipts must show the following purchase information: date, store name, item purchased, quantity of items, purchase price and payment confirmation.

Will I have to pay for shipping?

Standard shipping costs associated with sending the free bottle of HYLO® to you are covered by Candorvision. Free shipping may not apply to those located in remote or hard to reach areas. If you have questions about whether you qualify for this promotion, please contact us at patients@candorvision.com or (514)380-5270. Please note, you will have to pay postage costs for sending your redemption form, 10 barcodes and receipts to us.

How many free bottles can I receive through this program?

You can redeem a maximum of 4 free bottles through the 2020 Candorvision Patient Program. For each free bottle you wish to receive, you must send 10 original barcodes cut from the bottom flap of the retail boxes (showing the barcode, expiry date and lot number), the corresponding original proof of purchase receipts for each bottle as well as your completed redemption form. Purchases must have been made between January 1st 2020 and December 31st 2020.
