Purchase 10 bottles of HYLO®, HYLO®-DUAL, HYLO®GEL or HYLO DUAL INTENSE™ anytime between January 1st to December 31st 2022. Then, complete the redemption form here. You’ll get a confirmation email once you’ve completed the form. Then purchase 10 bottles of HYLO®, HYLO®-DUAL, HYLO®GEL or HYLO DUAL INTENSE™ anytime between January 1st to December 31st 2022. To get your free bottle of HYLO®, submit your 10 original barcodes cut from the bottom flap of the retail boxes (showing the barcode, expiry date and lot number), the corresponding original proof of purchase receipts for each bottle as well as your completed redemption form. Please check your junk/spam emails or add patients@candorvision.com to your contact list.
Collect ten (10) original barcodes by purchasing HYLO®, HYLO®-DUAL, HYLO®GEL or HYLO DUAL INTESE™ in Canada at valid retailers such as pharmacies or clinics between January 1st 2022 to December 31st 2022. Then, complete the redemption form here. Then mail the barcodes cut from the bottom flap of the retail boxes (showing the barcode, expiry date and lot number), the corresponding original proof of purchase receipts and your completed redemption form to: Candorvision (a div. of Candorpharm Inc.), P.O. Once we receive your submission by mail, you’ll get a confirmation email from us. We’ll send you your bottle of HYLO® as soon as possible, free of charge (Candorvision will cover the cost of shipping).
You’ll receive your free bottle of HYLO® as soon as possible after we receive your submission, within a maximum of 6-8 weeks depending on the volume of submissions. You’ll know we received your submission when we send you a confirmation email. Please check your junk/spam emails or add patients@candorvision.com to your contact list. To ensure your submission is processed as quickly as possible, please check the steps for how to get your free bottle before mailing in your submission.

This patient program requires the original bottom flap of the retail boxes (showing the barcode, expiry date and lot number) as well as original proof of purchase receipts and the completed redemption form. If you lost a receipt or bottom flap of one of your boxes, please contact us at patients@candorvision.com or (514)380-5270 before mailing your submission.
You may modify your receipts to hide payment information and other purchases, so long as any information relevant to this Patient Program remains visible. Receipts must show the following purchase information: date, store name, item purchased, quantity of items, purchase price and payment confirmation.

Standard shipping costs associated with sending the free bottle of HYLO® to you are covered by Candorvision. Free shipping may not apply to those located in remote or hard to reach areas. Please note, you will have to pay postage costs for sending your redemption form, 10 barcodes and receipts to us.
You can redeem a maximum of 4 free bottles through the Candorvision Patient Program. For each free bottle you wish to receive, you must send 10 original barcodes cut from the bottom flap of the retail boxes (showing the barcode, expiry date and lot number), the corresponding original proof of purchase receipts for each bottle as well as your completed redemption form. Purchases must have been made between January 1st 2022 and December 31st 2022.
